Use the following form to apply for accreditation with the Plumbing Industry Commission. When completing this form you will need to provide^* the following information:

• Applicant declaration
• The classes of plumbing work you wish to be accredited under
• Details of any other industry related accreditations or qualifications that you hold
• Experience statement(s) from your licensed supervisor(s) that detail your relevant experience gained
• Photograph if not previously provided
• Payment of the relevant application fee
• Each attachment should not exceed 1MB in size.
• It can take some time (30 seconds to 1 minute) to process after you submit, please be patient.
